Authorities say a hole in a ceiling and an open gate helped an inmate escape from the Oahu Community Correctional Center. 

The state Department of Public Safety on Friday released some details from the investigation into Daniel Skelton’s June 15 escape.

Department officials said Skelton climbed through an opening in the ceiling of a shower area. He jumped off the roof and walked through an open gate. Shielded by darkness and parked cars, he walked out of parking lot’s exit. 

The investigation is looking into possible staff misconduct.

Public Safety Director Ted Sakai said officials overlooked the ceiling and roof when making security upgrades. He said some policies weren’t followed properly.

Skelton was captured several days later. He was spotted running between homes in Nuuanu, sporting orange hair.
